Dear Oaks Health Family, 
We hope this message finds you well. Please note the following updates:

Medication Status: You can check your patient portal to see when/where and how your medications were sent in real time. Simply go to Menu on the upper right side, click "my medications" go to the medication that was prescribed or refilled and click on the name, a drop down section will show you that your med was sent via eRx or eFax with the name of the pharmacy and the date and time your provider sent your medication. If you do not see this under your meds, then you should reach out to your provider regarding your med status. 

Self Pay Rates:

Level 2- $55 (5-15 min or low complexity medical decision mkaing) 
Level 3- $65 (16-30 min or mod complexity medical decision making)
Level 4- $80 (31-45 min or high complexity medical decision making)
Any visit time above 45 minutes or with very high complex medical decision mkaing will result in a $200 charge. 

Weekend Provider Communication: If you need to speak with a provider over the weekend, please call or text the office at 919-336-2947 and leave a message. Portal messages are not conisdered urgent and may not be answered until the following week. 


Emergency Situations: If you are experiencing a medical emergency, please call 911 immediately. Do not wait for a repsonse from our office. But please send us a  message and let us know where you are so that we can connect with the ER. 


Important Appointment Reminder

Patients with chronic medical conditions (diabetes, high blood pressure, heart disease, stroke history, etc.) should be seen every 3 months with updated labs.

Patients being treated for ADHD, anxiety, or insomnia with medications must be seen every 6 months.

Please note: Refills will not be provided if you have not been seen within the required time frame.
